Drivers and conditions for chronic diseases in primary care in Sweden

In this project we are evaluating what the drivers and the conditions are to provide high quality care in the primary care setting for chronic diseases.

The Swedish healthcare is governed on regional level, with some functions on national level defining guidelines.

In the analysis we are evaluating:

  • How does the financing of primary care and responsibility for costs associated with care influence the conditions to provide good care for chronic diseases?
  • How are the results linked to primary care monitored? More information about this is found in the section “Outcomes that Matter”.
  • Is there any link between the health outcomes and financial impact?
  • And more.

We will share some data during the project as we engage with different stakeholders to validate the conclusions.
